Most men can't perform under pressure and regardless of what they say we like to feel wanted and confident. Start by telling him you want to go out on a date, bring it back to the way you were in the beginning, show him how fun it is just being with you again, send little flirty messages/texts. Get him ready for what is to come. Ask him what it is he likes or if he is willing to do something new. Are you willing to do something new. And if all else fails slip him a little blue pill.
